OJTs - On the Job Training

  1. Monthly OJT reports are due by the 10th of the following month.
  2. Make sure that all rows are added up in the final column.
  3. OJT reports must have all signatures to be accepted.

Apprentice Release Form

When an apprentice leaves the company, voluntarily or involuntarily, the employer must complete this form and submit it to the Apprenticeship Coordinator within seven (7) days.

Participating Employer Agreement

By signing this form, you agree to:

  1. Monitor your apprentice while he/she participates in the apprenticeship program.
  2. Abide by the annual wage survey.
  3. Review and sign off on the month OJT reports.
  4. Ensure that apprentices attend class and that their work schedules do not conflict with their class times.
